What is First Link® ?
First Link® is a program which connects individuals newly diagnosed with Alzheimer's disease or a related dementia and their families to support early and will continue throughout the course of the disease.
First Link® provides you with the opportunity to learn about every step in the Alzheimer Journey through the First Link® Learning Series.
First Link® connects you to Alzheimer Society Programs and Services and provides information about other community and health care services.
First Link® connects you with an opportunity for individual support or support groups.
Why refer to First Link® ?
• To get connected to the Alzheimer Society early in order to prevent crisis situations.
• To reduce caregiver burnout and pre-mature long term care placement.
Who can make a referral?
• Anyone can make a referral to First Link®
• Family physician, self, family members, community care partners, allied health professionals.
• How to make a First Link® referral
• Fax or e-mail a referral form (with the patient
or their family member's permission) to the First Link® Coordinator and we will make the connection from there.
• You will receive an acknowledgement letter once contact has been established.

First Link® Learning Series
• The First Link® Learning Series courses are progressive. Each course builds upon the other to provide participants with a comprehensive overview of dementia, coping strategies, resources and support systems. Participants have the opportunity to learn and share with others who are affected by dementia.
